Where should we start? There are so many different certificates and reasons why they are requested.
Some are very standard, and we can quickly generate them from your electronic medical records; others - are much more complex. We frequently need to use complex forms, making it much more time-consuming.
Unfortunately, they take time, even the simple ones. In our clinical practice, time and experience is the only "commodity", so there will be a small charge (the same as for scripts) for any additional certificates issued outside the consultation times.
For more complex forms and certificates, we will discuss with you the best way of obtaining them - preferably during our sessions so we can fill them together and you know the content before it is signed.
These certificates will be emailed to you, or you can pick them up from the office.

- If they are simple medical certificates - sickness, certificates for schools, university or other reasons where you require information about the diagnosis and treatment, we can generate them via your electronic records.
- If specific forms are required - some of them we have already programmed in our system - such as Medicare forms, Centrelink forms (standard certificates, but not individual Treating Doctors Reports), Health Department notification forms, and SCSA forms.
